Psychiatrist, author, and gender medicine dissenter
Dr. Stephen B. Levine has treated gender dysphoria since 1974 — longer than most current practitioners have been alive. A co-founder of one of America's first gender identity clinics and a former WPATH standards committee chair, he grew alarmed as clinical rigour gave way to affirmation. His conversation with Stella O'Malley offers Irish clinicians and parents a rare perspective from inside the history of gender medicine.
Stephen B. Levine is a Clinical Professor of Psychiatry at Case Western Reserve University School of Medicine in Cleveland, Ohio, where he has practised for more than five decades. He began treating gender dysphoria in 1974 and co-founded one of the United States' earliest gender identity clinics, accumulating a depth of clinical experience that is almost without parallel in the field. He has also published widely on human sexuality and the psychology of love, authoring several books that remain standard references for mental health professionals.
Levine served as chair of the committee that produced an early version of the World Professional Association for Transgender Health (WPATH) standards of care — the guidelines that shape gender clinics worldwide, including those now being developed in Ireland. Over time he became troubled by what he saw as the subordination of psychiatric rigour to ideological affirmation, and he stepped back from WPATH, becoming one of the most authoritative clinical voices calling for caution, thorough assessment, and informed consent processes that are genuinely informed.
That history matters enormously in an Irish context. Ireland is at a crossroads: the Oide guidelines, the HSE's developing gender services, and ongoing debates in schools and Dáil committees all demand the kind of grounded clinical wisdom Levine represents. When a man who helped write the original rulebook says the field has lost its way, Irish policymakers, parents and clinicians would do well to listen.
